Social Care and Health
Staff in Social Care and Health departments can assist in accessing many and varied services required for the person you care for. Services may include: personal care (washing, dressing etc.); meals in the home; respite, residential care; specialist transport and aids and appliances to assist independence at home(Charges can be made for these services depending on financial circumstances.)
Contact the Social Care and Health department and ask for a:
Community Care Assessment – for the person you care for. Social work staff will assess what services will be appropriate for the person and whether they are eligible for these services. Your views as a carer should be taken into account throughout the Community Care Assessment. Your needs will also be considered when services are being allocated to the person you care for. The Community Care Assessment may lead to services for you, which, in turn, will help with your own health and wellbeing.
Carers Assessment – as a carer you have a right to your own assessment. You are eligible for this even if the person you care for refuses a Community Care Assessment. For more information go to our Carers Assessment page
Care Plan/package – This will be based on the findings of the assessment, and will give information on what services Social Care and Health will provide and who’s paying for them.
Professionals who may work with you
- Social worker – offers help and advice, information and support to people and their carers. They will negotiate, plan and organise the care package/plan based on the assessment. They will also liaise with other professionals; ensuring that there is a comprehensive care package for you and the person you care for.
To obtain help contact your local Social Care and Health department (departments are usually separated into adult care departments and children’s services). If the person you care for is in hospital contact the Hospital Social Care Team.
If the care plan specifies help in the home someone from Social Care and Health will arrange this and monitor how arrangements are working.
Community care worker or care assistant - these staff come into the home to help with tasks such as helping people get up, personal care, cooking and shopping.

Social Care and Health Area Offices:
The Directorate’s Offices are the first points of call for all the Social Care and Health services. They are open 8.30am - 5.00pm Monday to Thursday and 8.30am - 4.30pm on Fridays.
An emergency service is available outside these hours to deal with high risk situations that cannot wait for the local office to open.
